Create your first FMEA

Create a document and complete step 1, planning and preparation.

Chapters · 3:20

This guide creates a new FMEA document and completes step 1, planning and preparation. What you set here goes straight into the report header and becomes the basis for every later step.

Start from the sample document

After you sign in, the home screen lists the documents you worked on recently. New accounts start with a sample document, My FMEA, inside My Project. It is a completed analysis of a wiper system, and looking around it first is a quick way to see how structure, functions, and failures connect.

Create a document

  1. 1Open a project from the list on the left.
  2. 2Click Add Document at the top right.
  3. 3Enter a name for the document. The analysis template is required and the default template is already selected; it is the format used when you export to Excel.
  4. 4Open the new document from the list. It opens on step 1.

Choose the FMEA type

The FMEA type comes first. Use Design FMEA to analyze a product design and Process FMEA to analyze a manufacturing process. Steps 2 to 7 stay locked until the type is confirmed.

  • Design FMEA subtype: system, hardware, or software.
  • FMEA-MSR supplement: turn it on for safety-related designs to also evaluate monitoring and system response.
  • Analysis levels: set the depth of the structure tree. The default is three levels, and a Design FMEA can go deeper.

Fill in the report header

Fields marked with an asterisk are required and are checked in audits. Empty required fields get an orange outline, and the badge on the right shows how many are filled in.

  • Company name, location, confidentiality level, report template, and rating criteria start from the defaults saved in your settings. Switch a field to Custom to use a different value for this document only.
  • Rating criteria are the scales behind the severity, occurrence, and detection scores of this document. They are attached to the Excel report as well.
  • FMEA scope: write down what is included and what is excluded. This boundary is the basis for the structure analysis in step 2.
  • Start date is filled in with the creation date. Revision date is filled in when you confirm a revision in the version history.
  • Team: enter the responsible department, the cross-functional team members, the tasks to perform, and the tools to use. This is where the AIAG-VDA 5T planning is documented.

Confirm and move on

There is no save button; everything is saved automatically. When step 1 is ready, click Proceed to Next Step. This confirms the FMEA type and unlocks steps 2 to 7.

The FMEA type cannot be changed after it is confirmed, so check it once more before you click.
