What is FMEA-MSR?
FMEA-MSR stands for Supplemental FMEA for Monitoring and System Response. It is an AIAG-VDA FMEA type for systems that can detect a fault and respond to it during customer operation — for example, a controller that detects a sensor failure and switches to a safe state. MSR analyzes whether the system’s monitoring and response keep the end effect at an acceptable severity, which is central to functional safety work such as ISO 26262.
Because it asks a different question than a Design FMEA, MSR uses a different scale: Severity, Frequency, and Monitoring (S/F/M) instead of Severity, Occurrence, and Detection. Frequency describes how often the failure cause occurs in the field, and Monitoring describes how effectively the system detects the fault and responds in time.
How MSR is rated
MSR has its own Action Priority logic. Following the AIAG-VDA handbook, when Monitoring is rated 1 — meaning the system is almost certain to detect the fault and respond in time — the severity used for prioritization is the mitigated severity, the severity of the outcome after the system has responded, rather than the original, unmitigated severity. For other Monitoring ratings, the original severity is used. QSA Hub captures the monitoring controls, the system response, and the mitigated severity, then returns the MSR Action Priority from the AIAG-VDA MSR tables. As with DFMEA and PFMEA, you can record optimization actions and re-rate while keeping the original values for traceability.
How MSR fits with the DFMEA
MSR is supplemental — it complements the Design FMEA rather than replacing it. The DFMEA covers how the design itself can fail; the MSR covers how the system detects those faults and responds during operation to keep the outcome safe. Safety and diagnostics engineers, particularly on ISO 26262 programs, use MSR alongside the DFMEA.
