FMEA (Failure Mode and Effects Analysis) is a structured, team-based method for identifying how a product or process can fail, evaluating the risk of each potential failure, and prioritizing actions to prevent or detect it before it reaches the customer.

What does FMEA stand for? (Full form)

FMEA stands for Failure Mode and Effects Analysis. A failure mode is the way something can fail; the effects are the consequences of that failure. The analysis rates each failure's risk and prioritizes what to fix first. You may also see it written as "FMECA" when a separate criticality analysis is added.

What is FMEA used for?

FMEA is used to catch potential failures early — while a design or process can still be changed cheaply — and to produce a documented, defensible record of how risk was assessed and reduced. In regulated industries such as automotive, it is also a customer requirement, typically following the AIAG-VDA standard.

How FMEA rates risk: S, O, D and Action Priority

Each potential failure is rated on three scales of 1–10 — Severity (S), Occurrence (O), and Detection (D). FMEA in quality, manufacturing & other industries

FMEA started in aerospace and the military and is now standard practice in automotive (under AIAG-VDA), medical devices, electronics and semiconductors, and general manufacturing. In quality management it supports failure prevention and continuous improvement, and it appears inside frameworks such as Six Sigma (DMAIC) and automotive APQP.

When should you use an FMEA?

Typically when you introduce a new product or process, make a significant design or process change, respond to recurring field or quality problems, or need to meet a new customer or regulatory requirement.
